I understand that a consultation with a Functional Therapist is completely voluntary, and under my own free will.
I understand that Functional Therapists do not diagnose conditions nor do they prescribe or perform medical treatments, prescribe substances, nor interfere with the treatment of a licensed medical professional. They may not offer advice on any operation or medical procedure or treatment or other medical intervention; nor may they offer a medical diagnosis or ‘cure’.
I acknowledge that the Functional Therapy is an aid to the healing of mind, body and spirit, which can complement any medical or psychological care I may be receiving.
I acknowledge that a Functional Therapist retains the right to refuse or postpone a consultation at any time if it is felt that such treatment is, according to his or her professional judgment, inappropriate for whatever reason (such as the client being under the influence of drugs or alcohol, or because they are acting in a threatening or otherwise inappropriate manner).
I understand that although my Functional Therapist will provide client care within a comfortable and safe atmosphere, s/he cannot be held responsible for theft or damage of any personal belongings, including vehicles parked outside a consultation room.
I understand that I remain fully responsible for my actions in my quest for well-being. I take full responsibility for physical, emotional and mental changes that may be sustained by me as a result of me using Functional Therapy.
I agree to hold Bertie Hanekom, Edgar Phillips, Perspective Training College, Functional Therapists, workshop organizers and property owners completely harmless of all liability for adverse effects, injury, harm, illness or damage (including relationship changes) alleged to have been caused by the use of the methods and techniques of Functional Therapy.
I acknowledge that I am unable to claim back from medical aid for any Functional Therapy consultation. I also understand that appointments must be cancelled 24 hours in advance or full payment is due.
